XORTX Therapeutics Targets FDA New Drug Application for XORLO as Gout Prevalence Projected to Surge

Gout, a painful form of inflammatory arthritis, is set to become far more common in the coming decades. Projections indicate that global gout prevalence will rise significantly over the next twenty-five years, driven by aging populations, rising obesity rates, and lifestyle factors. In the United States alone, over 9 million people have already been diagnosed with the disease, and an even larger group—44 million people—have uric acid levels above the normal range, placing them at risk. This widening patient pool underscores an urgent need for better treatment options, as current therapies often come with severe side effects that lead to poor adherence.

Existing gout medications, particularly oral xanthine oxidase inhibitors (XOIs), are the standard of care but carry boxed warnings for cardiovascular risk and cause gastrointestinal issues, kidney toxicity, and even initial flare-ups that deter patients from continuing treatment. Approximately 3% to 5% of patients cannot tolerate these drugs at all. This leaves a significant gap for safer, more effective therapies. XORTX Therapeutics Inc. (NASDAQ: XRTX), a late-stage clinical pharmaceutical company, believes it has a solution with XORLO™, a proprietary formulation of oxypurinol. According to the company, XORLO overcomes the historically poor oral absorption of oxypurinol by using a liposomal delivery system that enables high bioavailability and rapid absorption. This allows for effective uric acid lowering at substantially lower total daily doses, minimizing toxic-level exposure and severe gastrointestinal side effects associated with high-dose traditional therapies.

XORTX is making tangible progress toward bringing XORLO to market. The company recently announcing the launch of drug material and tablet manufacturing to support a planned two-part clinical study, XRX-OXY-102. Contract production of pure oxypurinol drug substance using XORTX’s proprietary synthetic pathway has already begun, and the company is working with its contract manufacturing partner toward commercial drug batches. This manufacturing milestone is critical because the clinical study requires GMP drug and clinical supply tablets representative of what will be submitted to the FDA for marketing approval.

Regulatory progress is equally important. XORTX is working toward a new drug application submission to the FDA for XORLO, with a new Investigational New Drug (IND) application prepared for submission in the fourth quarter of 2026. The IND describes the clinical development plan, and once the two-part trial is complete, XORTX intends to file a New Drug Application (NDA). If successful, the company will request a pre-NDA meeting with the FDA and prepare an NDA submission alongside pre-commercialization activities. XORTX is targeting an NDA submission in about one year and estimates a market opportunity of $700 million annually for XORLO.

“Our planned IND submission to the FDA next quarter is a defining step for the XRx-026 program,” said Allen Davidoff, Co-CEO of XORTX. “It reflects the strength of our prior clinical results and moves us closer to potentially offering gout patients a differentiated treatment option.” The implications of this announcement extend beyond a single company. With gout prevalence soaring and current treatments failing many patients, a new, better-tolerated therapy could significantly improve adherence and outcomes. For investors, XORTX’s progress toward an NDA represents a potential near-term catalyst. As the company continues to advance XORLO, it is positioning itself to address a large and growing unmet need in gout care. More information about XORTX can be found on XORTX.
